2010 Charitable Beneficiary Application
REQUIREMENTS
- Applications must be submitted on-line by February 19, 2010 to be considered.
- All questions must be answered or acknowledged to the best of your ability.
- The contact person must have the authorized ability to speak on behalf of the applying organization. The contact person may be asked to interview on behalf of the applying organization.
- All monies donated to your organization are to be used for the purposes written in this application.
- Your organization must regularly participate in scheduled events during the 2010 season.
- Your organization must participate in the recruitment effort of runners through your contacts, newsletters, members and those that support your organization's mission.
- Your organization will write and publish articles in newsletter, website or local paper about the Team M.A.D. fundraising efforts where possible.
- Approximately one year after receiving the grant, the organization will submit a follow-up letter indicating receipt of the grant and a narrative of what programs were supported and how such programs directly benefited the local community.
